If applicable, please print and bring the completed form(s) to your visit. For your convenience, they can be filled out on your computer upon downloading them.

No powder, cream, perfume or deoderant on breast or underarm area.

ULTRASOUND – GENERAL
Abdominal and/or gallbladder
Nothing to eat or drink 4 hours before the exam (unless emergency).
Pelvic or Obstetrical
Drink 8 oz. of liquid one hour prior to examination.
CT SCAN
Abdomen / Pelvis
Nothing to eat or drink 4 hours before the exam, unless otherwise instructed.
Head / Neck / Chest
Nothing to eat or drink 4 hours before the exam.
CT CORONARY CALCIUM SCORE
Nothing to eat or drink 4 hours before the exam.
